Generic Rescue Forms

The various rescue organizations that make up MAR do everything they can to ensure that the animals in their care go to a suitable home that is full of love, attention, and responsiblity. They become very attached to the animals in their care, so only the best possible homes will do. Any questions they ask will help them determine what animal best suits you. When you contact any of them about adopting, they realize that you will probably already have a specific animal in mind. But they would like to take every possible step to ensure that any animal that leaves their care STAYS in his or her new home. They always take back any of their animals, but it is a traumatic experience and if they can avoid that possibility, then they will try to make sure that each home is perfect! As such, each step of the process uses a different form which should be filled out and signed.

The following generic forms are in Word format. They are designed to be changed to suit any rescue organization.

Surrender/Animal Release

Whenever possible, rescue organizations try to accept animals from people who can no longer keep their pet for a various list of reasons. They understand that some people cannot take on the responsibility of a pet.

Since a person is asking the rescuer to take that responsibility from them, they must fill out and sign a surrender form. Although not always mandatory with the private rescues, a donation towards the cost of the pet's care would be very much appreciated when a person surrenders their pet. The various rescues may be non-profit organizations, but they are not exempt from paying the same bills that the owner pays. And a non-profit means just that: they don't make any money. Any monies that come in go directly to the care of the animals.

Foster Application

This is a contract between the rescue organization and the caretakers of the animal that is up for adoption.

Every year millions of cats and dogs are put to sleep in shelters, millions more die because they are abandoned and are left to fend for themselves. If you find these numbers deplorable you may want to be a temporary foster parent to homeless cats or dogs. Foster homes provide a temporary place for an animal where they can learn the social skills of living with a family. The more foster homes we have the more animals we can rescue.

The rescue organization provides the food, vet care, etc. You provide the TLC.

Foster homes are the life-blood for all rescue groups. Please, if you love animals consider becoming a foster parent. The rewards are great, it’s a lot of fun plus you do it at your convenience.

Adoption Application

The adoption decision depends upon many factors, including -- but not limited to -- the compatibility of the family and home to the individual animal and other applications received on the animal. It is the Rescue’s prerogative to decide which home is most appropriate for the individual animal. If a person is not selected for the animal they originally wanted to adopt, they may be considered for another animal.

Adoption Contract - Cat

Adopting a cat through rescue is a fairly simple process:

  1. Choose a cat, or cats, for adoption from the MAR Adoptable Cats page, Marni's list, or meet the adoptable cats at one of the shelters. You will be required to meet the cat(s) and the rescuer in person.
  2. Fill out the Adoption Application you will be given.
  3. If you are approved for adoption, you will need to fill out the Adoption Contract and pay an adoption fee (fees vary), which covers the cats spaying/neutering, testing and all vaccines.
  4. Enjoy and care for your new companion.

Adoption Contract - Dog

Adopting a dog through rescue follows the same simple process:

  1. Choose a dog, or dogs, for adoption from the MAR Adoptable Dogs page, Marni's list, or meet the adoptable dogs at one of the shelters. You will be required to meet the dog(s) and the rescuer in person.
  2. Fill out the Adoption Application you will be given.
  3. If you are approved for adoption, you will need to fill out the Adoption Contract and pay an adoption fee (fees vary), which covers the dogs spaying/neutering, testing and all vaccines.
  4. Enjoy and care for your new companion.
